Here's how:
Cut 6 x 6 timbers into four 4' lengths.

Notch.
With a circular saw, make a lengthways cutout 3" deep and 3" high. Start the cuts 3½" in from each end of 2 pieces of timber. These two pieces and the concrete slab will form the tabletop. Clean up notched end cuts carefully with a jigsaw and a chisel.

Through bolts.
Fasten each top piece of timber to another piece of timber to produce two sides. Bolt each pair together with ½" x 7" bolts, nuts and washers. Countersink bolt heads, nuts and washers 1" deep into the bottom and top pieces with a 1½" diameter forstner bit. Fasten the two table sides to the ends of a 2 x 6 spreader board. Centre the board and position its top flush with the bottom of the cutouts.

Concrete form for top.

Fasten a concrete form to the table frame. Construct the form with plywood and 2 x 4 boards.

Threaded rods installation.
Install two ¾" threaded rods, 16" in from the ends of the timber and centred 3/8" below the plywood. Drill 1½" x 1" deep countersunk holes and drill a ¾" hole all the way through. Install the threaded rods and snug them up. Install casters centred and 2" in from each end. Place wire fence in the concrete form.

Mix concrete and pour.
Tap edges to remove bubbles and smooth the top surface of the concrete. When concrete is cured, remove 2 x 4 sides and bottom pieces, and snug up ¾" rods.

Stain as desired.
